PRIMARY REQUIREMENTS
Passport valid at least 6 months
Clear scanned copies of passport stamps and visas. (Expired and valid)
Digital copy of Passport Sized Colored Photo (4.5 cm x 3.5 cm) taken from a photo studio
Original PSA Birth Certificate / Marriage Certificate valid for at least 1 year.
FINANCIAL REQUIREMENTS
Original Bank Certificate including type of account, opening date, ADB & current balance
Clear scanned copy of 6 months bank statement.
TR (Income Tax Return) Form 2316 for Individual / Form 1702 for Companies
ADDITIONAL REQUIREMENTS
FOR EMPLOYEES
Original Certificate of Employment / Certificate of Engagement / Contract
Clear Scanned Copy of ID
Pay slip / Invoices
Approved Leave / Travel Authority (if applicable)
FOR BUSINESS OWNERS
Clear scanned copy of Mayor's Permit, DTI or SEC include GIS of the corporation.
Clear scanned copy of BIR company registration.
FOR STUDENTS
Original copy of School Certificate of Registratior
School ID
Leave of Absence
FOR LICENSED PROFESSIONALS
Clear scanned copy of PRC Card or IBP Card
SPONSORED TRAVEL
Proof of relationship / connection e.g. Birth Certificate, Pictures, etc.
Clear scanned copy of Passport of Sponsor
Letter of Invitation
Statutory Declaration (if applicable)
